With reference to the "Accidents & violations reports & violating the requirements for issuing a professional CDWS card " documented in the CDWS board meetings , and in order to preserve the lives and safety of the divers and tourists and to sustain a high reputation of the Egyptian tourist destinations.
CDWS board have decided to impose the relevant penalties on the violating guides, instructors and technical managers in the Red Sea governorate according to the Penalties manual.
Therefore, you are kindly requested to review the position of each Snorkel Guide/ Dive Master/ Instructor through the link below in case it is active or still suspended. You can also search through the link by the number of the professional CDWS card or the name of the Snorkel Guide/ Dive Master/ Instructor.
Follows find below the list of the current violators and their penalties:
South Sinai Governorate
1- Mr. Ebrahim Zakaria Ebrahim - Diving instructor - CDWS professional ID card # 13287
The penalty after reduction: Suspension for 3 months and 15 days.
Violations:
• Violation of international standards by the absence of usable oxygen cylinder on board.
• Causing a serious accident due to negligence.
2- Mr. Ahmed Mohamed Elsayed - Diving instructor - CDWS professional ID card # 11301
The penalty after reduction: Suspension for 4 months.
Violations:
• The absence of participants documents, Liability Release, medical statement and the diving log signed by participants.
• Violation of international standards for Introductory diving (by carrying out Introductory diving activity without the required skills).
• Executing Diving Activities for his own account outside legal and licensed entities from ministry of tourism.
• Absence of first aid tools and emergency oxygen.
3- Mr. Gasser Sameh Fathy - Diving instructor - CDWS professional ID card # 15283
The penalty after reduction: Suspension for 3 months and 15 days.
Violations:
• Violation of international standards by not preforming a check dive to evaluate the participants.
• Causing a serious accident that leads to injuries which required hospitalization.
4- Mr. Hossam Ebrahim Attia - Diving instructor - CDWS professional ID card # 13309
The penalty after reduction: Suspension for 3 months and 15 days.
Violations:
• Violation of international standards for snorkel activity by indirect supervision (not being in the water with participants during the Execution of the activity) despite their emphasis on participants having an accompanying guide in the water.
• Violation of international standards for snorkel activity as the liability Release signed by customers in a language they do not understand.
• Causing a serious accident leading to death.
5- Mr. Amr Adel Hossny - Diving instructor - CDWS professional ID card # 13742
The penalty after reduction: Suspension for 15 days.
Violations:
• Violation of international standards by the absence of usable oxygen cylinder on board.
• Absence of the Covid-19 statement for snorkel participants.
• Providing first aid in a wrong way that is not internationally recognized.
6- Mr. Mohamed Ahmed Mohamed - Diving instructor - CDWS professional ID card # 12857
The penalty after reduction: Suspension for 15 days.
Violations:
• Violation of international standards by the absence of participants documents, Liability Release and medical statement for some participants with false names signed on the documents submitted.
7- Mr. Mohamed Ashraf Mohamed - Diving instructor - CDWS professional ID card # 15514
The penalty after reduction: Suspension for 4 months.
Violations:
• The absence of participants documents, Liability Release and medical statement signed by participants.
• Violation of international standards for Introductory diving (by carrying out Introductory diving activity without the required skills).
• Executing Diving Activities for his own account outside legal and licensed entities from ministry of tourism.
• Absence of first aid tools and emergency oxygen.
8- Mr. Mohamed Saad Mohamed - Diving instructor - CDWS professional ID card # 13219
The penalty after reduction: Suspension for 3 months and 15 days.
Violations:
• Marketing and selling Diving Activities for his own account outside legal and licensed entities from ministry of tourism.
• Violation of international standards for Introductory diving (by carrying out Introductory diving activity without the required skills and photographing participants while directly supervising them during Introductory diving).
9- Mr. Mohamed Aly Abd El-Rahman - Diving instructor - CDWS professional ID card # 13646
The penalty after reduction: Suspension for 2 months.
Violations:
• Violation of international standards by not confirming the presence of a usable oxygen cylinder on board.
• Causing a serious accident (as a result of the absence of usable oxygen cylinder for use on board).
• Violating the audit protocol by the absence of any of the boat crew or staff (non-professional) of the facility at the place of executing the activity holding a valid first aid certification.
• Absence of the Covid-19 statement for snorkel participants.
10- Mr. Mohamed fathy zein Elabedin - Diving instructor - CDWS professional ID card # 15208
The penalty after reduction: Suspension for 1 month and 15 days.
Violations:
• Causing an accident by taking a participant to a dive despite his knowledge of a health contraindication that may lead to a risk to the life of the participant and the absence of a report from a doctor that she was allowed to engage in an introductory diving activity.
• Violation of covid-19 precautionary procedures by using the same equipment with a number of participants without sterilization.
11- Mr. Mahmoud Samir Hussain- Dive Master - CDWS professional ID card # 12748
The penalty after reduction: Suspension for 15 days.
Violations:
• Violation of international standards by the absence of usable oxygen cylinder on board.
12- Mr. Hady Abd El-Azim Mohamed- Diving instructor - CDWS professional ID card # 11263
The penalty after reduction: Suspension for 7 months.
Violations:
• Violation of international standards by the absence of participants documents, Liability Release and medical statement for some participants with false names signed on the documents submitted.
• Violation of international standards by not conducting a risk assessment prior to activity and endangering the lives of participants by engaging in snorkel activity in an area where activity is not permitted in accordance with the instructions of the national park.
• Manipulating data and providing false names with official papers in order to avoid payment of the national park fees, failure to write the names of all participants on board and false of the rest data.
• Causing a serious accident.
• Violating the audit protocol by not writing all participants on board and false of the rest data.
• Violation of precautionary measures by not applying the ratios imposed to face the Corona pandemic.
13- Mr. El-Sayed Mohamed Abd El-Rahman- Diving instructor - CDWS professional ID card # 11892
The penalty after reduction: Suspension for 3 months.
Violations:
• Manipulating data or forms.
• Causing a serious accident.
• Violation of precautionary measures.
14- Mr. Hesham Sayed El-Dessouky- Diving instructor - CDWS professional ID card # 13921
The penalty after reduction: Suspension for 3 months.
Violations:
• Falsifying the necessary papers to obtain the CDWS card.
15- Mr. Alaa El-Din Youssef- Diving instructor - CDWS professional ID card # 12447
The penalty after reduction: Suspension for 8 months and 15 days.
Violations:
• Violation of international standards by the absence of participants documents, Liability Release and medical statement for some participants with false names signed on the documents submitted.
• Violation of international standards by not conducting a risk assessment prior to activity and endangering the lives of participants by engaging in snorkel activity in an area where activity is not permitted in accordance with the instructions of the national park.
• Manipulating data and providing false names with official papers in order to avoid payment of the national park fees, failure to write the names of all participants on board and false of the rest data.
• Assist and facilitate the work of illegal entities by dealing with street kiosks and websites unlicensed from ministry of tourism and Antiquities
• Causing a serious accident.
• Violating the audit protocol and the technical manager declaration by not writing all participants on board and false of the rest data.
• Violation of precautionary measures by not applying the ratios imposed to face the Corona pandemic.
16- Mr. Moaz Mohamed Metwaly - Diving instructor - CDWS professional ID card # 15264
The penalty after reduction: Suspension for 4 months.
Violations:
• The absence of participants documents, Liability Release and medical statement signed by participants.
• Violation of international standards for Introductory diving (by carrying out Introductory diving activity without the required skills).
• Executing Diving Activities for his own account outside legal and licensed entities from ministry of tourism.

The penalty: Suspension for 2 months.
Violations:
• Conducting inappropriate actions and behave in an unprofessional manner.
• Violation of international diving standards by carrying out courses without documented parental or legal guardian consent is obtained when the client is a minor.
18- Mr. Aly Ahmed Abd El-Hamid - Diving instructor - CDWS professional ID card # 15240
The penalty: Suspension for 1 month.
Violations:
• Violation of international diving standards by carrying out courses without documented parental or legal guardian consent is obtained when the client is a minor.
19- Mr. Aly Mohamed Ahmed Abd El-Halim - Diving instructor - CDWS professional ID card # 14022
The penalty: Suspension for 1 month.
Violations:
• Violation of international diving standards due to the absence of emergency oxygen and a first aid kit during the implementation of the activity.
20- Mr. Mohamed Ahmed Mohamed Hussain - Diving instructor - CDWS professional ID card # 12728
The penalty: Suspension for 1 month.
Violations:
• Conducting inappropriate actions and behave in an unprofessional manner by submitting a malicious and unfounded complaint without any support or evidence.
21- Mr. Tamer Rabie Hussain - Diving instructor - CDWS professional ID card # 10478
The penalty: Cancellation of the CDWS professional card.
Violations:
• Conducting an inappropriate action by attacking the CDWS auditor verbally, or offensively.
Red Sea
1- Ms. Agnieszka Winka - Diving instructor - CDWS professional ID card # 12885
The penalty after reduction: Suspension for 1 month and 15 days.
Violations:
• The absence of Liability Release.
• Causing an accident as a result of negligence.
• Violation of international standards of recreational diving (by exceeding the depth limit of recreational diving and endangering the lives of participants).
• Failure to report the accident at the time of its occurrence.
2- Mr. Ashraf Mahmoud - Diving instructor - CDWS professional ID card # 10408
The penalty after reduction: Suspension for 4 months.
Violations:
• The absence of Liability Release.
• Violation of international standards of recreational diving (by exceeding the depth limit of recreational diving and endangering the lives of participants).
• Causing an accident as a result of negligence, putting the life of the participants at risk.
• Manipulating records by writing depths contrary to reality in the dive log.
• Failure to report the accident at the time of its occurrence.
3- Mr. Mohamed Abd El-Aziz Mohamed - Diving instructor - CDWS professional ID card # 14115
The penalty after reduction: Suspension for 1 month and 15 days.
Violations:
• Conducting inappropriate actions and behave in an unprofessional manner.
• Damage and breaking down coral reefs by sitting on them.
4- Mr. Khaled Mohamed Dawood - Diving instructor - CDWS professional ID card # 10392
The penalty after reduction: Suspension for 1 month and 15 days.
Violations:
• Violation of international standards for snorkel activity (violation of safe supervision ratios with 17 participants and one snorkel guide).
• The absence of participants documents, Liability Release and medical statement and the activity log.
• Causing a serious incident due to a profession error.
5- Mr. Mohamed Mohamed Nagib - Diving instructor - CDWS professional ID card # 12785
The penalty: Suspension for 1 month.
Violations:
• Violation of international standards for snorkeling activity due to the absence of a medical declaration and liability release for snorkeling participants and implementation of the activity despite the presence of a yes answer to the participants medical declaration.
6- Ms. Nadia Mohamed Moghazy - Diving instructor - CDWS professional ID card # 15718
The penalty: Suspension for 6 months.
Violations:
• Marketing and executing trips for her own account without a licensed diving center from the ministry of tourism and antiques.
You are kindly requested to act in accordance with the board of directors’ decree by banning the above-mentioned persons to work/operate during their period of suspension, in order to avoid the penalties stated in the law no. 85 of year 1968 regarding the violations of the decisions of the CDWS board.
